I agree to the Membership Agreement and understand I may be required to sign additional liability releases to use the Zen Vitality facilities. I also consent to recieve email and text communication about my appointments, membership, or marketing communication from Zen Vitalis LLC.

I acknowledge that payment for my selected service will be due at the time of my first visit to Zen Vitality.
